Fetal Monitoring
Academy
Clear, accurate, and unbiased evidence about fetal monitoring - all in the one place.
Supporting you to support women's informed decision-making.
Yes! I want instant accessEver experienced something like this?
Nora saw a maternity professional yesterday, and was told that because she is planning a vaginal birth after caesarean section she MUST have continuous CTG monitoring and can only use the shower or bath if a telemetry monitor is available. Nora called you as she questions whether this information is correct. She prefers to have another monitoring option, but doesn't want to sacrifice safety at the same time. She wants your advice about her options for fetal monitoring.
Now what?
- You want to be sure the information you share is absolutely based in quality research and supports safe decisions.
- You know you need to tread carefully as the maternity professional might make life difficult for you if you provide misleading advice.
But...
- You don't have the time, research skills, or access to an academic library to read and understand all the relevant research yourself.
The evidence you need to confidently tackle this conversation, and any conversations about fetal monitoring practices, is waiting for you inside the Fetal Monitoring Academy. Come join Dr Kirsten Small and find other people who are working to make experiences like Nora's a thing of the past.

Fiona - Midwife
I joined the Academy because I wanted to be able to share evidence-based information around options for fetal monitoring, particularly if they found themselves birthing in a hospital setting where the use of CTG's is commonplace. To have the evidence so thoroughly explained and summarised has been extremely helpful and I have incorporated this information into the antenatal classes I teach so that families can be empowered to question guidelines and policies if they wish.
Bertine - Student Midwife
As a student midwife I felt there was so much I didn't know about fetal monitoring while being aware that CTG monitoring is "required" at many if not all hospitals for birth whilst little evidence is known. The Academy is a great place to learn: self explanatory and nicely structured to follow on your chosen pace, leaving room to adjust to you schedule with other lessons, exams and internships. It has been very helpful in accumulating knowledge and using it to counsel clients & patients and supporting their choices!
Barb - Midwife
I tell every midwife and labour & delivery nurse I know about Dr Kirsten Small and Birth Small Talk. I find the Academy so empowering and in line with my lived experience as a maternity care provider.

Take A Peek Inside
Here's some of what you will find inside the
Fetal Monitoring Academy
☑️ Understanding research: Build your skills in reading and understanding any research. Includes research maths, statistics made simple, and why forest plots are fun!
☑️ What went into the Cochrane review: Understanding the context in which the research was done provides important insights into what the results show. Find out which trial breached their randomisation protocol, which ones had unethical approaches, and some big "wait, what?" moments from the Dublin trial.
☑️ What about women with risk factors?: A detailed look at what the research really says about using CTG monitoring for women at increased risk. This module will have you rethinking everything you have been taught before!
☑️ And many more: Twelve complete and comprehensive modules are available, and more are coming in the near future - including one specifically about fetal monitoring in the context of vaginal birth after caesarean section.

Anna Melamed - Midwife and University Lecturer
Here's what Anna has to say about the Fetal Monitoring Academy.
Is the
Fetal Monitoring Academy
right for you?
This is for you if..
- You work with women who are pregnant or have recently given birth
- It is your goal to provide accurate, unbiased information about fetal monitoring
- You believe women can be trusted to make good decisions and their decisions should be respected
- You want to join a community of like-minded people working to make maternity care safer and more respectful
This is not for you if..
- You aren't interested in applying research evidence to your clinical or educational practice
- You prefer to stick with comfortable mistruths and myths
- You don't believe women should make decisions about their maternity care
- You don't want the current maternity care system to change as you think it is working just fine the way it is
Only 9% of women report being given both information and a choice about fetal monitoring. Many maternity professionals and educators simply do not know what research says regarding fetal monitoring in labour. Don't let this be you...
Anyone who works with women who are pregnant, giving birth, or who have given birth, and who is having conversations with them about fetal monitoring will find something really valuable waiting inside the Academy. This includes people who are registered health professionals (nurses, midwives, obstetricians) and those who are not (like doulas and childbirth educators). Students, researchers, and people who work in policy and quality improvement roles are also welcome and will find lots of value here. If you are simply a giant birth nerd and want to learn more - there's a place here for you too!
What you won't find in this course
Many maternity professionals are required to complete mandatory fetal monitoring study sessions. These typically include long lists of indications for CTG use, definitions to describe the different fetal heart rate patterns, and actions to take when the patterns are not "normal". But they rarely touch on the evidence about whether CTG monitoring has been proven to be superior to intermittent auscultation (or not), what the harms of different fetal monitoring approaches are, and what the evidence tying heart rate patterns to particular outcomes really shows.
This is NOT your typical fetal monitoring course. You'll find all the bits you should be learning, but that no one else is teaching, inside the Fetal Monitoring Academy.

What else do I get?
- To make sure you don't feel lost and overwhelmed by the sheer amount of information waiting inside the Fetal Monitoring Academy, I created the Seven Steps to Success program. Keep on the right path and track your progress, earning badges as you achieve each step! Or- forge your own way through your particular interests - it is completely up to you!
- Short, end-of-module quizzes to help you self-assess whether you "got" the important learning points.
- Certificates of completion as you finish each quiz, so you can track your learning time. These can be used as evidence if you are required to complete professional development hours.
- All modules are fully referenced so you can double check all the information for yourself.
- Downloadable resources to support your practice.
- Optional monthly Challenges in the Common Room provide you with a gentle and supported nudge to help make maternity care better in your local area.
Hi - I'm Dr Kirsten Small
I'm a retired obstetrician with a PhD in fetal monitoring. I'm an experienced researcher and educator. I have devoted much of the past decade to understanding, building, and sharing the evidence about fetal heart rate monitoring.
Now I use what I have learned to help you to make maternity care better. I'll share with you the detailed facts about what works, what harms, why, and what we might do to make things better.
